I am a UK taxpayer and would like you to claim back tax on this donation and all I make in the future until I notify you otherwise. (You must have paid an amount of Income Tax and/or Capital Gains Tax at least equal to the tax that the charity reclaims on your donation (currently 25p for each £1 you give).

Please download the Gift Aid Declaration form here and fill it in, print and send it to the address included. Please note the GiftAid is only fulfilled when the declation is filled out and sent.

Refund Policy: If you make an error in your donation please advise Friends of Amma within 14 days and a full refund will be made to you by cheque. Please note that payment will only be made to the name that appears on the credit or debit card.
